How do I get to segment the interest region as shown in a red line region? I already try using many methods but failed to segment those region. Thank you.

I tried many clustering methods but still cannot get the whole region. It is because of the intensity is not homogeneous. For reference, clot5.png is original image, and the Roi_soft.png is a ground truth Thank you.
