This allows you to return the orientation, major axis length, and minor axis length of binary data with regions that approximate ellipses.

Regionprops performs its operations for each connected region of true values, so it treats that cluster of three pigs as a single entity.
The way you do this likely depends on the range of scenarios that you expect from your images. I would suggest looking through the functions in the Image Processing Toolbox, particularly those that relate to Segmentation to see which ones might be useful.
