Simulink SR flip flop giving output 1 even though the input are 0 0 and the previous state is also set at zero.

I tried to reproduce the issue at my end, unfortunately I am getting the expected output. Please find below screenshot for the same.

I would recommend that you debug your model in a stepwise manner and check if the previous state of your SR flip flop is changing to 1.
If this dosen't resolve your issue, in order to better assist you kindly share your Simulink model file.
