How to segment skin rashes, sores and pimples ?

I want to segment skin rashes, sores and pimples on hand and mouth. The segmented image should include only rashes, sores or pimples not other parts of skin.

I have attached the images that have rash, sores and pimples on it.

Any help specially providing the code is greatly appreciated.

I have tried thresholding, edge detection, kmeans and morphological but did not get the desired result.
