how to read the optical encoder counts from Arduino Nano 3.0 to simulink?

Hello,
I connected my optical encoder to the arduino, phase A to pin2, phase B to pin3, (the encoder port number is LPD3806-400BM-G5-24C, it has 4 wires: VCC, GND, A phase and B phase)and then read its counts by coding the arduino, but I don’t know how to transfer this count to simulink in real time (not into matlab, I want to build in simulink)?
Thanks :D
